COMPARISON OF THE NON-STEROIDAL ANTIINFLAMMATORY DRUG INDUCED UPPER GASTROINTESTINAL TRACT BLEEDING VERSUS OTHER CAUSES IN ELDERLY PATIENTS

To compare the demographic characteristics, clinical and laboratory presentations, the need of blood transfusions, characteristics of endoscopic lesions and prognosis in NSAID Induced upper gastrointestinal bleedings versus other causes of bleeding in the elderly. 92 of 172 patients records aged above 65 years and hospitalized between January 2000 to January 2001 in intensive care unit were enrolled to the study. Patients were grouped as NSAID (+) (50 patients, 54%) and NSAID(-) (42 patients, 46%). Levels below 10 gr/dl were determined as a criteria for blood transfusion. Statistical analysis were done by using Chi-square and T- tests. Demographic, clinical and laboratory parameters were FIM: 0.72 and 0.82, average age 76.9 ± 10.2, and 76.8 ± 9.5, melana 92 %, 88 %,hematemesis 24%, 29%, arterial blood pressure 123 ± 45 / 172 ± 35 mmHg, 125 ± 47 / 169 ± 32 mmHg , pulse 88 per minute, 90 per minute, Hb 9.8 gr/dl, 10.0 gr/dl, Hct 29%, 30%, urea 77 mg/dl, 66 mg/dl, requirement of blood transfusion 2.5 Unite, 2.9 Unite, length of hospital days 5.8, 6.9 , mortality 8%, 21%, endoscopic diagnosis gastric ulcer / gastric erosions 7/15 case, 5/12 case, duedonal ulcer / duedonal erosions 7/29, 8/7 in NSAII (+) and NSAII (-) groups respectively. Endoscopic examination could be performed 97% of patients. There were no differences between two groups related to these parameters (P>0.05). Comorbid diseases such as hypertension, diabetes mellitus, ischemic heart disease, congestive heart failure and malignencies had similarly distribution ranges each groups. Although between two groups by using the parameters such as low hemoglobin levels, more blood transfusion and more gastric lesion in NSAII (+) group than NSAII (-) group there were no significant differences Keywords : GIS, Bledding, NSAİİ
